The worst time to find out your documentation is not audit-ready is during an audit. The second-worst time is the week you get the notice. The best time, obviously, is before either of those has happened.
What follows is a pragmatic checklist of the documentation most small and growing teams need in place before they come under outside scrutiny - whether that scrutiny arrives as an inspector visit, an insurer questionnaire, an enterprise customer's security review, or a tribunal request.
Everything here is framed qualitatively, because audit criteria vary by sector, country, and specific body. None of this is legal advice. What it is, is a starting point: a set of record categories that repeatedly matter, and a sense of what “good” looks like in each. Consult a qualified advisor for your jurisdiction.
1. Training records: who was trained on what, and when
Auditors and inspectors typically want to see, for each member of staff, evidence that they received training relevant to their role, that the training covered the topics it was supposed to cover, and that the training happened within a sensible timeframe.
A training record that holds up usually has:
- The person. Identifiable by name and role, ideally with an employee ID or equivalent.
- The training.Specific topic, not a generic “completed induction.” If it was induction, what was covered.
- The version.Which version of the training they completed. This matters because content changes over time, and “completed in 2023” against a 2026 revision is not quite the same thing.
- The timestamp. When completion happened, ideally captured by the system rather than self-reported.
- Evidence of completion.A knowledge-check score, an e-signature, a supervised sign-off. Something beyond “they said they watched it.”
The pattern to avoid is the spreadsheet of “we trained everyone on this.” An auditor reading that sees one uncorroborated claim. An auditor reading individual records with timestamps and knowledge-check results sees something harder to challenge.
2. Policy acknowledgments: signed, timestamped, traceable
Policies are the organisation's commitments. Acknowledgments are the evidence that each staff member has received and accepted those commitments.
For commonly requested policies - data protection, safeguarding, health and safety, acceptable use, code of conduct, anti-bribery - you typically want:
- The version acknowledged. Which version of the policy the person saw. When a policy updates, a fresh acknowledgment cycle typically starts.
- The signature. Electronic signatures are widely accepted. A typed name in a Word document with no audit trail is not really a signature.
- The timestamp. Server-side, not user-side. When the signature was captured.
- The IP or device context. Additional metadata that links the signature to a real act by a real person. Captured automatically by most modern systems.
The ICO's published expectations on demonstrating accountability under UK GDPR are a useful reference point for the tone here: demonstrate, do not assert. Asserted compliance is weak evidence.
3. Procedure adherence evidence: proof the work happened
This is the category most teams are weakest on. You have an SOP. You have trained people. But can you show, for any specific day, that the procedure was followed?
Useful evidence at the procedure level commonly includes:
- Completion logs. A dated record that the procedure was run, by whom, and when each step was completed.
- Photo proof on steps that matter. Temperature readings, equipment condition before and after, finished setups. Photo-attached proof is fast to capture and hard to fabricate.
- Readings and measurements.Where the procedure requires a specific value to be recorded, the value itself is the evidence. Not “checked,” but the actual reading.
- Notes and exceptions. When something did not go as expected, what was the issue and what was done about it. Silence on exceptions reads, to an auditor, as exception-blindness.
The pattern to avoid is procedures that are thoroughly documented but never verified in execution. An SOP with no execution evidence is at best a guide to what the process should be, not a record of what it was. In most auditor interactions, that distinction matters.
4. Incident records: what, when, what you did
Incidents are going to happen. Auditors rarely expect zero incidents; they expect evidence that incidents were recognised, recorded, investigated, and acted on.
A usable incident record typically contains:
- Description.What happened, in specific terms. Not “an issue.” A person in role X was affected by Y at time Z.
- Immediate action. What was done in the moment, by whom.
- Investigation. Who looked into it, what they found, what the root cause was judged to be.
- Corrective action. What changed as a result. A new procedure, an extra step, additional training, an equipment repair.
- Follow-up. Evidence that the corrective action was actually implemented and that the change held.
HSE's guidance on incident investigation is a reasonable reference for the level of detail expected in a thorough record, particularly for safety events. The principle generalises: an incident record that stops at “investigated” and does not show corrective action will often be challenged. An incident record that shows a named change, with evidence it was implemented, typically holds up.
The corrective-action loop is the part most teams skip.An investigation that concludes “more training required” and never shows the training being delivered and evidenced is incomplete. The loop closes when the new training has records of its own.
5. Certifications and renewals tracker
For roles that require specific certifications - food safety, first aid, DBS, specialist qualifications, statutory inspections - you need a live tracker that shows what is valid, what is due soon, and what has expired.
A defensible tracker commonly has:
- The certification itself. A copy, or a reference to the issuing body and number.
- The holder. Named person, with role context.
- The valid-from and valid-until dates. Explicit, not implied.
- A renewal lead-time. When the organisation starts the renewal process before the expiry date. Leaving it to the last week is a common pattern and a common cause of gaps.
- A contingency plan. What happens to the work if a certification lapses unexpectedly. Is there another certified person on shift? Does the task have to stop?
FSA and sector-specific bodies publish clear expectations on certification record-keeping for regulated activities. The tracker does not need to be sophisticated; it needs to be current and accessible.
6. Access control and change records: who did what
For any system that holds operational records - training platform, SOP library, incident log, certification tracker - auditors commonly want to see who had access, who made changes, and when.
Useful properties of a system from an audit perspective include:
- Named user accounts.Not shared logins. If “admin” could be any of five people, every entry under that login is effectively unattributed.
- Role-based permissions. Who can view, who can edit, who can approve. Evidence that permissions are scoped appropriately is commonly expected under security frameworks.
- Audit logs. A record of who changed what, when. Particularly important for any change to a published policy, SOP, or training record.
- Version history. For documents, a trail of what changed between versions, by whom, and why.
The ICO's expectations under UK GDPR around access control and change logging for personal data are a useful benchmark. They generalise well beyond personal data: the same controls that protect personal information also protect the integrity of your operational records.
7. Supporting records that often come up
Alongside the six categories above, a handful of supporting records regularly get asked for. You do not need a separate system for each - but you do need to know where they live.
- Risk assessments. For activities that warrant them, including the assessment, the mitigations, and the review date. HSE typically expects written risk assessments for anything non-trivial in a workplace of five or more people.
- Contracts and supplier records. Who you buy from, what the agreed standards are, what evidence you hold that suppliers meet them.
- Complaints and feedback records. What was raised, what the response was, what was learned.
- Equipment and maintenance logs. For equipment that requires periodic maintenance or inspection, evidence that it happened and that the equipment is safe.
- Right-to-work and onboarding checks. Evidence that standard pre-employment checks were completed, relevant to jurisdiction.
Common weaknesses, and how to close them quickly
Most small and growing teams have some version of each category above. The weaknesses tend to cluster in a predictable way. If three of these sound familiar, they are usually the fastest wins.
The training record is a spreadsheet. It shows completion but not evidence. The fix is to start capturing knowledge-check results and e-signed acknowledgments alongside the completion claim. Retrospectively for key roles if possible; from now on as a habit.
Policies exist but acknowledgments do not. There is a Policies folder. No one can tell who has read what, or when. The fix is a one-time acknowledgment cycle on all current policies, then a standing rule that every published policy triggers a fresh round.
SOPs are documented but execution is not. You can show auditors the SOP for closing the kitchen. You cannot show them the closing log for last Tuesday. The fix is to add completion logs with photo proof on the two or three highest-stakes SOPs first, and expand from there.
Incidents are logged but corrective action is not tracked to closure. The investigation happens, a corrective action is named, nobody checks whether it was actually implemented. The fix is a short monthly review of open incidents and their corrective actions, with explicit closure evidence before anything gets marked complete.
Certifications live in someone's memory. The person whose certification is about to lapse usually does not know it is about to lapse. The fix is a single view with expiry dates and lead-time alerts. Spreadsheet-with-conditional-formatting is fine as a starting point.
What readiness actually feels like
An audit-ready team is not a team with perfect records. It is a team that can answer any of the following in under five minutes, with the supporting evidence to hand:
- Who is trained on what, as of today?
- Which policies are current, and who has acknowledged each?
- Was this specific procedure followed on this specific date?
- What incidents have we had in the last twelve months, and what did we change as a result of each?
- Which certifications are valid, which are due for renewal, and who is responsible for each?
- Who made changes to this document, and when?
If you can answer all six in minutes rather than days, you are in a stronger position than most. If you cannot answer any of them quickly, you know where to start.
The hidden benefit is that audit-readiness and operational readiness are largely the same thing. The records that satisfy an inspector are the records that also let you run the business well - faster onboarding, clearer responsibility, tighter follow-through on incidents.
